We are seeking a skilled and compassionate Nurse Navigator to join our team. As a key member of our healthcare delivery team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities:
* Employ advanced clinical and professional practice capabilities to deliver high-quality patient-centered care.
* Operate from community care settings based at Gladstone, Biloela, Emerald, and Rockhampton.
* Provide education and support to patients and families to promote health literacy and self-management.
Required Qualifications and Skills:
* Current registration as a Registered Nurse with AHPRA and a current annual practising certificate.
* Relevant postgraduate qualifications or working towards one would be highly regarded.

Total Remuneration Package:
The total remuneration package includes an hourly rate between $66.85 and $72.58 per hour, as well as employer superannuation contributions and leave loading.
