Permanent Full Time position based between Rockingham and Mandurah
Rocky Bay are looking for a Support Coordinator to join our growing dynamic team on a permanent full time basis. This team delivers coordination supports to NDIS customers.
About the role
Work with customers to strengthen their capacity to connect with informal mainstream and funded supports
Develop customer capacity to maintain support relationships resolve service delivery issues and participate independently in NDIS processes
Be a part of a diversified and progressive disability service organisation

What you need to have
Experience in a similar role within the NDIS
Excellent communication and relationship-building skills
Proven ability to work collaboratively with customers stakeholders and team members
Personal resilience working in a changing environment
