Physiotherapist in Home and Community Care
This is a full-time position as a Physiotherapist in our team. The successful candidate will have a customer base in the North Metropolitan area.
* We are seeking an experienced Physiotherapist who can conduct comprehensive assessments, develop personalized care plans, and implement effective strategies to meet each client's unique needs.
* The successful candidate will design both individual and group exercise programs to keep clients motivated and engaged to optimize their function and independence.
* This role involves assessing and prescribing equipment to support client autonomy and well-being.
* We require a Physiotherapist who can foster collaboration and successful outcomes by communicating effectively with clients, families, and caregivers.
* Seamless care coordination and accurate progress tracking are essential for this role.
Requirements:
* Current FULL registration with AHPRA as a Physiotherapist (essential).
* The ability to relate to elderly customers with empathy while working effectively with other health professionals and service providers.
* Excellent interpersonal and communication skills are required.
* Proven ability to assess, plan, implement, and evaluate quality care.
* A current Australian driver's licence and reliable vehicle are necessary for this role.
