About the Role
Join our team as a Team Leader – Proactive Management and Support on a full time, fixed term role until February 2025 covering parental leave.
We are seeking a passionate, experienced Allied Health Clinician with excellent communication skills to join the Proactive Support and Management leadership team. The Team Leader performs a key clinical leadership role in supporting the program outcomes and management of staff as directed by the Program Manager, to achieve the goals and directions set out by Monash Health and Department of Health.
This role is integral to bridging the gap between acute and community services. It requires experience within acute settings as well as an understanding of community services. The role works across various Monash Health sites. We operate from a person‑centred care framework and align with the social model of health. As part of the role there is a requirement to travel to other Monash Health sites, so you’ll need a valid driver’s licence.
