Key Responsibilities:
* Conduct comprehensive assessments to develop tailored physiotherapy treatment plans for individuals in their own homes through Support at Home/NDIS.
* Collaborate with multidisciplinary teams to deliver holistic patient care.
* Educate patients and their families on healthy living practices, empowering them to thrive within their communities.
* Maintain effective communication with service providers to ensure seamless care delivery.
* Work autonomously to prioritize tasks, manage records, and provide high-quality patient reports.
Requirements:
* Bachelor of Physiotherapy or equivalent qualification.
* Registration with AHPRA as a practicing Physiotherapist.
* Police Check Clearance for working with vulnerable populations.
* Excellent verbal and written communication skills to engage with diverse clients.
* Strong organizational and time-management skills to meet deadlines.
* Ability to work independently, prioritizing tasks, and maintaining attention to detail.
