The MCCN provides care, information, and support for people with cancer across the continuity of care pathways and multidisciplinary teams across the cancer care community within the Health Service/demographic region.
The MCCN will provide collaborative clinical and supportive care for people and their families, along with the facilitation of communication across the multidisciplinary cancer care team. The MCCN will have expert knowledge regarding the spectrum of cancer care, inclusive of health assessment, treatment pathways, referrals, and on-going facilitation of clinical and supportive cancer care. The role provides leadership that facilitates the ongoing development of clinical practice that reflects the McGrath Model of Care.
The MCCN provides expert clinical advice to patients/clients, carers and other health care professionals with complex healthcare needs within Cancer Services. The CNC is a clinical expert who, through leadership, facilitates excellence in research based clinical practice, education, research and professional leadership.
The MCCN develops, facilitates implementation and evaluates care management plans for patients/clients diagnosed with specific cancers with a focus of care coordination on patients identified with complex needs. The Clinical Nurse Consultant will also develop education and training programmes, and participate in collaborative research with other health professionals in relation to Cancer Care Services.
The MCCN will provide expert consultation and advice to health care workers for patients with special or complex needs through their continuum of care until discharge, acknowledging that patients may require follow up or contact after discharge, ensuring support is provided to the patient from diagnosis through treatment, to discharge or terminal care.
The MCCN will establish and maintain communication with all relevant Medical Practitioners, Nursing and Allied Health Professionals, will assist in the organisation and participate in the multidisciplinary team meetings, and maintain a close liaison with all Division of General Practitioners, Community GP's, and Community health services.
The MCCN will facilitate and coordinate the scheduling of treatments, liaising with all members of the multidisciplinary team. The position will triage patient needs and make appropriate and timely referrals to allied health and support services. The CNC will assist and provide expert advice on matters associated with complex care management and discharge planning and provide information and advice tailored to meet individual patient and carers needs.
The MCCN will actively participate in the multidisciplinary team and will be involved in treatment planning. Ensure specific data required by the Foundation is added to the relevant McGrath Foundation controlled database on a regular and timely basis and attend and actively participate in McGrath Foundation education and information sharing events.

* Current AHPRA Registration
* Minimum five years full time equivalent post registration experience, with at least three years recent full time equivalent experience in the specialty field of Oncology, Palliative Care or Cancer Stream specific nursing or related discipline and post graduate qualifications relevant to the field of practice
* Valid NSW Drivers Licence
This is a temporarily funded position until 27 June 2027with the possibility of an extension subject to funding.
