Our organisation is committed to providing high-quality Support Coordination services, taking a person-centred approach to each participant we work with. We are seeking a locally based, experienced Support Coordinator to join our team.
The primary objective of this role is to provide high-quality Support Coordination for NDIS participants in the Perth region. You will be required to attend face-to-face meetings, participate in case conferences, assist with overseeing and managing NDIS budgets, and support your participants in successfully implementing their NDIS plans.
You will focus on strengthening a participant's ability to navigate intersecting service systems, including Health, Justice, Mental Health, Housing, and other Social Services. The successful applicant will contribute to the development of evidence-based and innovative clinical services that promote the delivery of high-quality Support Coordination.
Job Requirements
* Qualifications in Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology, or another Allied Health field
* Previous experience supporting participants with mental health, psychosocial, or disability support needs in a case management-style role
* Extensive knowledge of disability, mainstream, and community services
