This is an exciting opportunity for a Registered Nurse to join our team and deliver high-quality nursing care in the comfort of patients' homes. As a Hospital in the Home (HITH) nurse, you will be part of a small, supportive team that works across a flexible 7-day roster offering both 12-hour and 8-hour shifts.
About the Role
You will work alongside a multidisciplinary team including community nursing, palliative care, cardiac & respiratory rehabilitation, continence services, dietetics, diabetes education, wound management, and audiometry. In this diverse and rewarding role, you will deliver hospital-level nursing care in patients' homes, supporting their independence and wellbeing.
Key Responsibilities
* Deliver high-quality nursing care in patients' homes
* Work collaboratively with a multidisciplinary team to achieve patient outcomes
* Assess and plan patient care using evidence-based practice and critical thinking
* Develop and implement individualized care plans that meet patients' needs
* Provide education and support to patients and their families
Requirements
* A highly skilled and experienced Registered Nurse
