We are seeking an experienced professional to join us in Wagga Wagga, NSW, as part of the Psychosocial Physical Health Pilot Program. This role involves coordinating care for individuals with complex mental health and chronic physical health needs.
Your key responsibilities include:
* Coordinating care for individuals with complex mental health and chronic physical health needs
* Using a person-centred approach to support participants, families, and carers in developing and achieving recovery-focused goals
* Delivering health and wellness education to consumers and the broader community
* Facilitating community sessions and anti-stigma initiatives
To be successful in this role, you must possess:
* A Registered or Enrolled Nurse with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA, or a qualified Aboriginal Health Worker
* Strong clinical skills, including comprehensive assessments, care coordination plans, and safe and responsive practice
* A recovery-oriented mindset and solid understanding of recovery-based approaches for individuals with severe and persistent mental health challenges
* Knowledge of chronic care and proven competencies in supporting individuals with chronic health conditions
