About Mawarnkarra Health Service (MHS)
Mawarnkarra (MHS) is the largest primary health care provider providing direct services to Aboriginal and Torres Strait Islander peoples living in the Roebourne area. MHS provides the families of Roebourne with a fully functional and accredited Clinic that treats approximately 3,000 people per year.

The Position
We are looking for a Male Community Connector who will aim to engage, inform and assist people living in Aboriginal and Torres Strait Islander remote communities through the National Disability Insurance Scheme (NDIS) pathway process. The Duties
* The ability to assist in promoting broader community understanding and awareness of the National Disability Insurance Scheme.
* The ability to act as a cultural broker between the community and the Customer by providing relevant information and referring people with disability and families to the Support Coordinator.
* Assist community members to test their eligibility for the NDIS and support the Participant Pathway process.
* Assist in linking people for Customer meetings and provider appointments.
* Support local linkages with the local health clinic, community school, and other community based service organisations.\
* Support the Customer to assist people interested in the scheme to obtain and complete Access Request (ARF) forms and to attend appointments.
* Be available on-site to support Customer staff during scheduled visits.
* Advise the Customer of community and cultural events that may impact community visits.
* Assist Customer staff to locate potential participants.
* Be a point of contact for the Customer.
Essential Criteria
* Have full working rights in Australia
* NDIS Worker Screening Check or willing to obtain
* Current Western Australian drivers license
* Consent to undergo a WA Working With Children Check and a National Police Clearance
* Must hold a Passenger Transport Driver Authorisation (PTD) or consent to undergo the medical and subsequent application with the Department of Transport. Employment is conditional pending the outcome of this Authorisation.
* Current CPR certificate
* Able to pass regular and random drug and alcohol testing
* Attend and participate in the professional development program and relevant external seminars and conferences, as approved.
* Live locally or be willing to relocate.
