About the Role:
The Clinical Nurse is a critical member of the multi-disciplinary team at our organisation, working to provide high-quality care and assessment services to frail aged clients.
Key Responsibilities:
* Conduct comprehensive assessments in hospital or community settings, adhering to Commonwealth guidelines and the Aged Care Act 1997
* Collaborate with external health-related organisations to ensure seamless client care
* Develop and implement individualised care plans that prioritise client health and wellness
Required Qualifications and Skills:
* Registration with the Queensland Nursing Council
* A current practicing certificate
* Maintain registration requirements with the Australian Health Practitioner Regulation Agency (AHPRA)
* Advanced communication and negotiation skills, particularly when interacting with the public health system
