Community Care Leaders play a vital role in ensuring older Australians receive the right care at the right time.
This hands-on leadership position guides a team of passionate Home Care Workers to deliver high-quality, person-centered care that upholds dignity, independence, and quality of life.
Key Responsibilities:
1. Needs Assessment: Conduct thorough assessments of client needs to identify areas for support and develop effective care plans.
2. Care Planning: Collaborate with clients, families, and healthcare professionals to create personalized care plans that meet their unique needs and goals.
3. Service Coordination: Coordinate services to ensure seamless delivery of care, including arranging transportation, managing medications, and providing emotional support.
4. Advocacy and Education: Advocate on behalf of clients to ensure their rights are respected, and educate them and their families about available resources and support services.

Requirements:
