- Corryong Health (CH) is a public sector agency and one of six multi-purpose services in Victoria. Corryong Health is funded by Commonwealth and Victorian State Government funding, and provides a broad range of hospital, residential aged care, primary health and community support services.
- About the role:
- Responsibilities include:
- Conduct comprehensive nutrition assessments, develop client-centered nutrition care plans, and provide individualised interventions in line with current evidence-based practice and Dietitians Australia standards.
- Monitor and evaluate outcomes of nutrition care through regular follow-up, weight tracking, and documentation.
- Provide education and counselling to patients, residents, carers, and families, supporting long-term behavior change and chronic disease management.
- Collaborate with the Hospitality Services team to ensure meals are safe, nutritionally adequate, culturally appropriate, and aligned with consumer preferences and clinical needs.
- Monitor and act on changes in residents' weight and nutritional risk using validated screening tools and escalation pathways.
- Identify and respond to risk and feedback from clients, families, and staff in a timely and professional manner.
- Provide education and training to staff as appropriate, including food service staff, nursing teams, and allied health peers.
- Meet eligibility criteria for full membership of Dietitians Australia (DA).
- Meet the eligibility criteria for the Accredited Practicing Dietitian (APD program
- Effective and highly developed interpersonal skills
- Computer skills and the ability to provide relevant reports and statistics
- Willingness to undertake annual vaccinations, drug and alcohol testing, fit testing and employment checks (including Police Check and NDIS Worker Screening check) as required by Victorian law and Department of Health guidelines,
