Enrolled Nurse/Care Partner – Rockhampton
About the Role
* Coordinate and oversee in‐home care services
* Conduct care assessments, reviews, and service planning
* Collaborate with clients, families, and the care team to ensure individual needs are met
* Monitor care delivery to ensure quality, safety, and compliance
* Support carers and staff with guidance, training, and updates
* Willing to travel Rockhampton and surrounding areas (company car provided)

Qualifications
* AHPRA registered and practicing
* Driver's licence (access to a reliable vehicle with comprehensive insurance)
* Aged Care / Health Care related certificates
* Proven experience in aged care, disability, or mental health within a community setting
* Robust understanding of Aged Care and Disability Standards, as well as consumer‐directed care principles
* Current First Aid, CPR certification, and National Police Check
* Working With Children (WWC) check or willingness to obtain
Preferred Skills
* Knowledge of Support at Home (previously HCP) or NDIS (desirable)
* Robust communication and interpersonal skills
* Relationship‐building skills
* Problem‐solving and critical thinking
* Attention to detail and documentation accuracy
* Knowledge of community/aged care systems
* Leadership and team coordinationTech‐savvy and system competence
