About the Role
A Care Support Officer is required to join our team in Shepparton, Victoria. The successful candidate will conduct assessments, develop and maintain individualised care plans, and champion resident-centred care practices.
Key Responsibilities
* Conduct assessments with care staff and leadership.
* Design and implement best-practice care plans reflecting individual needs and values.
* Undertake reviews alongside the Facility Manager.
* Ensure assessments are completed within 72 hours of admission and updated as required.
* Consult with residents, families, nurses, and external providers for care planning.
* Lead continuous improvement initiatives through audits and feedback loops.
* Support carers and deliver emotional, social, and clinical support to residents.
* Manage specialised care areas including mobility, nutrition, wound care, pressure care, continence etc.
* Maintain thorough documentation and communication across care teams and families.
* Complete checks and coordinate required training with staff for new colleagues.
Essential Skills and Attributes
* Registered Division 1 Nurse with current National Practicing Registration.
* Excellent problem-solving and care planning capabilities.
* Strong working knowledge of the needs of elderly people residing within a residential care facility.
* Exceptional problem solving, assessment, and care planning skills.
* Excellent operational knowledge.
* Self-motivated, proficient with computerised documentation systems and records.
* Strong interpersonal skills with the ability to build and maintain working relationships within a multi-disciplinary team.
