About the role
Our single goal is to provide a service that builds 'healthier rural people and thriving communities', and in this role, you'll grow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their own communities.

In this role, you will provide physiotherapy care to patients in the community with a focus on minimising hospital admissions. You will work with patients in the acute or ED wards, Hospital in the Home (HiTH) and where intervention may prevent hospital admission. Clinical areas will include post illness, fall and surgical therapeutic interventions, and supporting respiratory, cardiac and pulmonary rehabilitation.
You will join a team of highly skilled and passionate Allied Health Professionals, and will collaborate closely with the Medical Ward, Ambulatory Care & HiTH Teams. You will also be connected to the Bathurst Physiotherapy Department, which is a friendly & supportive team with a wealth of clinical knowledge and the Allied Health Community Outreach for Healthy Ageing Team.
