Job Overview
This role focuses on enhancing the lives of older adults by connecting them with suitable mobility, strength, and balance programs and services. As a Specialist Support Worker, you will be guided by a program Case Manager to facilitate client participation in these activities, fostering confidence and safety in accessing their community.
Main Responsibilities:
* Support clients in engaging with mobility, strength, and balance activities, ensuring a safe and confident experience.
* Determine and provide access to appropriate activities considering social, logistical, and financial factors.
* Offer ongoing support and mentoring to individuals, caregivers, and other relevant parties as needed.
* Promote health and wellbeing concepts, enabling individuals to self-manage their health and mobility effectively.
* Facilitate connections to relevant community resources addressing identified needs.
* Collaborate with the Case Manager and other staff members to achieve client goals efficiently.
