360 Health + Community is a person-centred health service providing care and support to people when, where and how they need it most. Our teams deliver programs across Western Australia to help body and mind including mental health for young people at headspace Fremantle. Our vibrant team is currently seeking a 2 part-time (0.8FTE) Access Clinician's to join us in providing integrated, holistic healthcare and support.
As Access Clinician you will effectively engage with young people aged years to provide and coordinate care. Being one of the first contact points for young people accessing the service, Access Clinicians build a positive rapport with a wide range of young people, as well as their family and friends. The role involves assessing a young person's needs including complex risk assessments and developing action plans to mitigate any identified risk. Access Clinicians receive consistent support by participating in regular clinical supervision, and presenting and discussing Young People's status and progress with the clinical care team.
To be successful in joining our team, you will need:
* Tertiary qualification and professional registration in Psychology, Social Work, Occupational Therapy, Counselling or other relevant field
* Current Working with Children Check and National Police Certificate less than 6 months old (or be willing to apply)
* Ideally, experience working with youth mental health, LGBTQIA+ identifying people, First Nations people, or Culturally and Linguistically Diverse people
* Knowledge of and ability to provide evidence based, developmentally appropriate individual and group counselling and clinical interventions including single session therapies
* Excellent communication skills to build rapport with stakeholders, actively encourage the young person to participate in decisions relating to their own care, and present information at clinical meetings
