The Role
We are seeking experienced and compassionate Clinical Care Coordinators to join a government-aligned mental health service based in Darwin, Northern Territory. In this vital position, you will provide short-term, goal-oriented clinical case management to individuals and families experiencing complex psychosocial and mental health challenges.
This role plays a key part in ensuring that clients with unmet needs are supported through coordinated, trauma-informed, and recovery-focused care.
Key Responsibilities
Deliver high-quality, evidence-informed clinical services including risk assessment, brief intervention, care coordination, and referral for clients with complex needs.
Maintain accurate and confidential client records within the Client Management System, including progress notes, care plans, risk assessments, and other documentation.
Coordinate with a network of health, welfare, and community service providers to ensure integrated and responsive client care.
Apply trauma-informed, recovery-focused, and stepped-care approaches in all aspects of service delivery.
Contribute to service intake processes and quality improvement initiatives to support consistent, high-quality outcomes.
Participate in qualified supervision and Continuing Professional Development (CPD) to maintain clinical registration and best-practice standards.
Operate within a government-aligned service framework, upholding ethical, legislative, and professional requirements.
Qualifications and Requirements
Full registration with AHPRA as a Psychologist, Occupational Therapist, or Mental Health Registered Nurse, OR
Qualification as a Social Worker eligible for membership with the AASW and able to apply for Mental Health Accreditation.
