The Role
We are seeking experienced and compassionate Clinical Care Coordinators to join a government-aligned mental health service based in Darwin, Northern Territory. In this vital position, you will provide short-term, goal-oriented clinical case management to individuals and families experiencing complex psychosocial and mental health challenges.
This role plays a key part in ensuring that clients with unmet needs are supported through coordinated, trauma-informed, and recovery-focused care.
Key Responsibilities
* Deliver high-quality, evidence-informed clinical services including risk assessment, brief intervention, care coordination, and referral for clients with complex needs.
* Maintain accurate and confidential client records within the Client Management System, including progress notes, care plans, risk assessments, and other documentation.
* Coordinate with a network of health, welfare, and community service providers to ensure integrated and responsive client care.
* Apply trauma-informed, recovery-focused, and stepped-care approaches in all aspects of service delivery.
* Contribute to service intake processes and quality improvement initiatives to support consistent, high-quality outcomes.
* Participate in professional supervision and Continuing Professional Development (CPD) to maintain clinical registration and best-practice standards.
* Operate within a government-aligned service framework, upholding ethical, legislative, and professional requirements.
Qualifications and Requirements
* Full registration with AHPRA as a Psychologist, Occupational Therapist, or Mental Health Registered Nurse, OR
* Qualification as a Social Worker eligible for membership with the AASW and able to apply for Mental Health Accreditation.
