Essential Criteria:
* Qualification: A bachelor's degree in Social Work
* Registration: eligibility for membership of the Australian Association of Social Workers.
Selection Criteria:
* Demonstrated understanding of the psychosocial issues associated with acute and chronic illness.
* Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex case management and discharge issues.
* Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders throughout all organisational levels.
* Demonstrated well developed time management skills with excellent written and verbal communication skills including report writing skills and high level computer literacy.
* Demonstrated ability to be agile and adaptable and provide high quality clinical services across a broad range of clinical areas at short notice.
* Demonstrated understanding of issues associated with working with a culturally and linguistically diverse client group.
* Demonstrated commitment to professional development, evidenced based practice, and continuous improvement.
* Current unrestricted NSW Driver Licence (or equivalent).
