We are seeking a highly skilled and compassionate AOD Case Worker to join our team.
About the Role
The successful applicant will provide comprehensive case management and support to participants, helping them achieve their recovery goals. This includes developing and monitoring case plans, managing risks, and conducting drug and alcohol testing in line with program requirements.
You will work collaboratively with diverse communities and partner organisations, applying your knowledge of AOD and mental health to empower participants and facilitate meaningful, lasting change.
Key Responsibilities
* Develop and monitor individualised case plans that address the unique needs of each participant.
* Provide ongoing support and guidance to help participants achieve their recovery goals.
* Manage risks associated with substance abuse and develop strategies to prevent harm.
* Conduct regular assessments and reviews to ensure participant progress.
* Collaborate with other service providers to ensure a coordinated approach to care.
Requirements
