**Overview of the Role:**
The SAFER Nurse works in partnership with an interdisciplinary team to provide participant-centered healthcare, using a harm reduction and trauma-informed approach. The role requires a Registered Nurse, Registered Psychiatric Nurse or Licenced Practical Nurse with experience working with individuals with high levels of poly-substance use and mental health challenges.
**Key Responsibilities:**
* Provide care and support for individuals with complex primary care health needs and active poly-substance use.
* Work collaboratively with the SAFER team to develop and maintain individualized care plans.
* Collaborate with participants to enhance understanding of their health status and navigate through various services and community supports.
* Participate in staff meetings, trainings, educational forums, and engagement in community building and networking.
**Qualifications and Skills:**
* Registered Nurse, Registered Psychiatric Nurse or Licenced Practical Nurse.
* Minimum 1 to 5 years' full scope, outreach nursing experience working with people with high levels of poly-substance use and mental health challenges.
* Demonstrated commitment to social justice principles and practices, including harm reduction, anti-racism, decolonization, and combating stigma related to drug use.
* Experience working in solidarity with diverse populations, including people who are criminalized for their drug use.
* Strong analysis of colonialism and intergenerational impacts of historical and ongoing colonization.
