Support at Home Manager – Full-time
Dhauwurd-Wurrung Elderly & Community Health Service Inc. The DWECH Support at Home Manager is responsible for the overall management, administration, and coordination of Support at Home (SaH) care services with the aim to keep clients safely and independently in their homes. The role also includes working with other wrap around services, both internally and externally, where appropriate.
This role involves managing, guiding and developing Support at Home care employees, ensuring compliance with regulatory requirements, managing budgets, and maintaining client focused care in line with the DWECH strategic plan.
About the Role
•Lead and manage a multidisciplinary team delivering Support at Home services.
•Oversee care planning, service coordination, and client outcomes.
•Ensure compliance with the Aged Care Act and provider registration requirements.
•Monitor service quality, manage budgets, and report on performance.
•Engage with clients and families to promote consumer-directed care.
•Implement digital systems for care coordination and reporting.
What we are looking for
•A degree or diploma in health, aged care or social work is highly desirable.
•Additional qualifications in business administration or leadership would be an advantage.
•Familiarity with the Aged Care Quality Standards and upcoming industry changes well regarded.
•Minimum of 3 years of experience in home health care, community care, or a related field, with at least 2 years in a managerial role.
•National Police Check or the willingness to obtain within employer specified timeframe.
•Working with Children Check or the willingness to obtain within employer specified timeframe.
•Current Victorian Driver’s Licence.
•Proof of vaccination status consistent with public health orders and with DWECH policy.
