About the Role
Help older adults regain independence after hospital discharge by providing community-based occupational therapy. This temporary part-time role involves visiting clients in their homes, conducting functional assessments, developing tailored treatment plans, and coordinating equipment and home modifications. You will work closely with a multidisciplinary team and contribute to the Commonwealth-funded TACP program.
In This Role, You Will
Provide community-based OT to older adults following hospital discharge.
Complete in-home functional assessments and develop tailored, person-centred treatment plans.
Recommend and coordinate equipment and simple home modifications to maximise safety and independence.
Work closely with multidisciplinary colleagues to ensure smooth, safe transitions of care.
Support clients to improve function, build confidence and achieve the goals that matter most to them.
Contribute to a high-quality, Commonwealth-funded TACP program that truly makes a difference.
About You
You enjoy working with older people and are good at putting them at ease. You are practical, down-to-earth, and able to adapt quickly to what each home visit brings. You communicate clearly, build strong rapport with clients and families, and are confident making clinical decisions in the community with a supportive team behind you.
AHPRA registration as an Occupational Therapist.
An interest in aged care, rehabilitation and community-based practice.
Strong assessment, communication and documentation skills.
Confidence working independently in client homes while staying connected with the team.
A current driver's licence and reliable transport.
