Classification: Registered Nurse Level 3 Grade 1 (PN / Health Professional Level 4/ Psychologist Level 4
Note: This role will be filled at either Health Professional Level 4, Psychologist Level P4 or Registered Nurse Level 3 Grade 1 depending on qualifications and experience, in line with the Enterprise Agreement.
Salary for HP4: $125,344 - $139,120 (Plus 12% Super)
Salary for P4: $133,437 - $140,285 (Plus 12% Super)
Salary for RN3G1: $126,196 - $131,177 (Plus 12% Super)

About the Role:

As a Care Navigator with the Liaison and Navigation Service (LaNS), you'll be at the forefront of transforming care for people with complex health and psychosocial needs across the ACT. You'll work within a passionate multidisciplinary team, guiding consumers and their supports through the maze of health and community services, ensuring they receive the right care, at the right time, in the right place. This is your chance to champion person-centred, trauma-informed, and strengths-based care, while building meaningful relationships with GPs, acute and community services, and external agencies. You'll coordinate treatment, advocate for consumers, and help shape the future of this innovative service through hands-on clinical support and co-design.
