Care Manager (Grade 5 RN) – Memory Support Unit
Location: Warralily Gardens, Armstrong Creek, VIC.
Hours: Monday to Friday | 7:00 AM – 3:00 PM (or variation depending on operational needs)

We are seeking a hands-on, caring, and passionate Care Manager (Grade 5 RN) to lead our Memory Support Unit at Warralily Gardens, Armstrong Creek.

Located on the doorstep of the beautiful Surf Coast in Geelong, our aged care residence in Armstrong Creek – ‘Warralily Gardens’ – is a modern and vibrant residential aged care home. Central to the lively local community, Warralily Gardens proudly offers a comprehensive array of senior care services, catering to the diverse needs of older people seeking quality nursing home and aged care options in the Geelong and Surf Coast region.
TLC Aged Care Armstrong Creek has an extensive range of on-site services available making TLC Warralily Gardens a unique place to live, including a community medical centre, allied health services including a hydrotherapy pool, gym, physiotherapy, dietetics and podiatry, a resident’s sports bar and café, and virtual reality cinema.
What You’ll Be Doing
As a Care Manager, you will provide exceptional dementia care, working closely with a high-performing team to ensure a safe and supportive environment for residents. Your key responsibilities include:
· Managing medication administration & wound care
· Driving continuous clinical improvement for excellence in care
· Supporting residents, families, and visitors with professionalism
· Ensuring timely assessments, care plans, and reviews
· Leading clinical education & staff competency training
· Fostering a positive and collaborative workplace culture

· Minimum two years’ experience in a clinical leadership role
· Strong understanding of Aged Care Quality Standards
