The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

About the Position
The Specialist Liaison Officer is responsible for supporting specialist clinics through the coordination of client screening, ensuring patient safety, and facilitating specialist and allied health appointments. The role supports the planning, engagement, and coordination of Derbarl patients' attendance and follow up appointments for Specialist Clinics. This includes working closely with clinical teams to ensure continuity of care and timely access to specialist services.
The position requires effective liaison with internal staff, external clinical specialists, hospitals, and other providers of clinical health services to support integrated and coordinated care. This role contributes to a culturally appropriate and positive health program aimed at improving health outcomes and wellbeing for Aboriginal people. Specialist clinics supported by this role include Optometry, Cardiology, Respiratory, Dermatology, Endocrinology, Nephrology, and other specialty services. The role offers a varied and engaging multi‐disciplinary work environment aligned with chronic disease management and coordinated patient care.
Key Responsibilities
* Act as the first point of contact for clients, providing initial screening, vital signs assessment, and clinical support as directed by GPs and Specialists.
* Coordinate specialist and allied health services, including booking appointments, arranging transport, and liaising with external providers, government agencies, and NGOs.
* Support delivery of chronic disease management, health assessments, and care planning, encouraging client participation in ongoing care.
* Provide pre‐ and post‐consultation follow‐up in line with Medicare requirements and ensure continuity of care through appropriate referrals and monitoring.
* Undertake clinical support activities such as wound care (as directed) and assist in implementing client management plans.
* Maintain accurate, timely documentation and manage client information using Communicare, including recalls and data entry.
* Advocate for clients and assist individuals and families to access health and community services, ensuring culturally appropriate and person‐centered care.
* Ensure informed consent processes are followed and uphold confidentiality, safety, and quality standards in all interactions.
* Collaborate with internal teams and stakeholders to support coordinated service delivery and positive client outcomes.
* Contribute to quality improvement, risk management, and a strong workplace health and safety culture.
* Participate as an active team member, demonstrating effective communication, professionalism, and commitment to continuous professional development.
* Undertake other duties within scope, as directed by the Specialist and Allied Health Coordinator.
Selection Criteria
* Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act.
* Current Registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner or Enrolled Nurse.
* Certificate IV or Diploma in Aboriginal Health or Enrolled Nurse.
* Knowledge of current health issues, impacting Aboriginal Health.
* Understanding the principles of public health including health education, health promotions and disease prevention.
* Well‐developed written and verbal communication skills.
* Knowledge and understanding of the holistic approach to health service provision.
* Experience working in a multi‐disciplinary team environment.
* Demonstrated ability to maintain confidentiality and security of records and information.
* Knowledge of Work Health & Safety, Industrial Relations and Equal Employment Opportunity Legislation.
Appointment Pre‐requisites
* Current WA Driver's license
* Satisfactory National Police Check which is not older than 6 months
* Willingness to undergo Pre‐employment Health Assessment
