As a Support at Home Service Partner, you will be the primary point of contact for our participants in our local LGA´s. The role requires exceptional communication, organisational skills with experience working in the aged care sector.
You will be responsible for facilitating participant access to services and providing administration support to the Support at Home team. You will liaise with our Support at Home participants, internal and external stake holders, and suppliers to ensure our participants care needs and goals are met within appropriate time frames.
You will be responsible for:
* Providing high level of customer support to Support at Home participants and team
* Liaising with Support at Home participants and all involved in their care and internal and external providers to arrange and manage support services in the participant’s home and community
* Providing timely follow up of referrals and service requests to facilitate optimal care provision for participants to services
* Scheduling and rostering of services into Carelink (client management system) and documenting all actions and liaisons
* Coordination of appropriate services internally and externally and scheduling of appointments
* Develop and maintain relationships with service providers to ensure service coordination and all relevant participant information is obtained accurately
* Monitoring and maintaining the after hours On-Call phone (weekly rotation paid)
* Working from a range of DPV Health sites and in the community
To be considered for this rewarding role you will have:
* Qualification in community, aged, health or another discipline desirable
* Knowledge of & enthusiasm for Community Aged Care Services & Consumer Directed care
* High level problem solving, exceptional customer service skills and analytical skills
* Professional telephone manner
* A strong customer focused approach to care
* Knowledge of Carelink + or other client management software
The fine print
* Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.
* Level 2 First Aid Certificate
