Key Responsibilities:

- Provide counseling interventions, including harm reduction strategies and complex case management, to support clients in achieving their goals.
- Establish and maintain relationships with community partners and other service providers to ensure seamless delivery of care and support services.
Requirements:
- Relevant tertiary qualification in health or welfare field (e.g., Social Work, Psychology, Counselling, Nursing).
- Proven skills in assessment, case coordination, counseling, and case management.
- Ability to work collaboratively with clients, families, colleagues, and stakeholders.
