Home Care Partner
We are seeking a highly skilled and compassionate Home Care Partner to join our team.
The role involves coordinating care for clients in their homes, ensuring they receive the best possible support to live safely and independently.
* Key responsibilities include:
* Managing client caseloads to develop close working relationships and focus on proactive care.
* Identifying client needs and responding appropriately with empathy, objectivity, respect, and professionalism.
Requirements include:
* A tertiary qualification in health, allied health, or social science and minimum two years' experience in care management.
* Demonstrated understanding of the needs and challenges faced by ageing individuals wishing to remain at home within their community.
* Experience in delivering person-centered and holistic care management.
