About the Role

We’re expanding our dedicated team on the beautiful Central Coast, and we’re looking for someone just like you ! Someone who brings clinical expertise, professional integrity, and a warm heart to every client interaction.
As a Community Registered Nurse, you will play a critical role in assessing, planning and delivering personalised clinical care to our elderly clients in their own homes. You’ll work closely with a supportive multidisciplinary team, contribute to best-practice care planning, and help ensure our clients can live independently, safely, and with dignity.
Key Responsibilities
- Conduct clinical assessments of Home Care Package (HCP) clients
- Develop individualised, evidence-based care plans
- Provide high-quality clinical nursing care (e.g., wound, continence, catheter, medication management)
- Deliver client education and promote reablement
- Collaborate with GPs, allied health and family to ensure holistic care
- Complete documentation, referrals, and clinical reporting
- Guide and support Community Support Workers to align service delivery with care plans
- Monitor, evaluate and escalate care needs as required
- Perform cognitive assessments and provide behaviour support strategies for clients
About You
We’re seeking a Registered Nurse who is caring, organised, and clinically confident, with a passion for aged care and a proactive, can-do approach to community health.
- Current AHPRA Registration (RN) with minimum 2 years of clinical experience
- Strong assessment and care planning skills
- A current driver’s licence and your own reliable vehicle
- Willingness and ability to travel throughout the Central Coast region
- Excellent time management and the ability to work independently, and as part of a team
- A commitment to client-centred care and clinical excellence
