About the Role Grampians Health Ballarat Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC PYP) provides Allied Health, Nursing and Centre Based Social and Respite Groups for people to assist them to remain living at home and in their community. This new role is an exciting opportunity to develop a nursing service with a focus on memory support, and to join our multidisciplinary CHSP and HACC PYP teams. Key Responsibilities Provide specialist and holistic care to clients and their carers living in the community in relation to their: mild cognitive impairment (MCI) dementia other cognitive changes impacting function chronic and complex needs Act as a resource person and provide education on management of behavioural & psychosocial symptoms of cognitive change to clients, carers & colleagues. Clinical assessments for Centre Based Groups and Cottage Respite Skills & Experience Registered Nurse Division 1 with AHPRA and hold a current practicing certificate. Post Graduate qualifications in a relevant field Demonstrated knowledge of and/or experience in contemporary nursing working with older people, people with dementia and people with complex needs.
