The Child and Family Care Navigator is a critical role in the multidisciplinary team at the Head to Health Kids Hub. This position requires strong communication and interpersonal skills, as well as the ability to work with diverse stakeholders.

Key Responsibilities:

1. Build rapport and trusting relationships with children and their families, extending to extended family members when necessary.
2. Assist families in navigating complex systems, particularly mental health and local services.
3. Maintain proactive contact with children and families throughout involvement with the Kids Hub, addressing their needs on an as-needed basis.
4. Provide assessments and case management for children accessing the Kids Hub, ensuring coordinated and personalized support.
5. Advocate and promote the wishes, thoughts, and opinions of children and families, offering choices of interventions and services while providing referrals as required.
6. Facilitate engaging workshops designed to support and enhance the mental health and wellbeing of children.
7. Provide ongoing liaison, negotiation, consultation, and relationship building with Multi-Disciplinary Team members and other key stakeholders.
8. Ensure support to children and families is provided in a culturally appropriate manner.
9. Participate in multidisciplinary team case review meetings, facilitate case tracking processes, and develop Individual Care Plans.
10. Assist the Community and Partnership Manager with community education, professional training, presentations, and reporting on statistical data.
11. Maintain effective contact records for each client and participate in the evaluation and development of the Kids Hub model.
12. Ensure the safety, health, security, and wellbeing of children within your care.

**Requirements:**

* A valid WA driver's license.
* A Working With Children Card or willingness to apply for one.
* A Department of Communities Check.
* National Police Clearance.
* Proof of working rights.

**Essential Skills and Experience:**

* A minimum Certificate 3 in Community Services or relevant discipline, or extensive experience working within a community setting.
* Demonstrated experience and knowledge of trauma-informed practice in relation to children and families.
* Demonstrated empathy and sensitivity when working with vulnerable children and families.
* Ability to form trusting relationships with diverse stakeholders from within a child's ecological system.
* Demonstrated experience and knowledge of conducting intake assessments and providing warm referrals.
* Demonstrated ability to advocate amongst professional peers on behalf of children and young people.
* Experience in providing supportive and psychoeducation-based interventions.
* Understanding and knowledge of referral pathways for children and their families within the mental health system.
* Experience working with people from culturally diverse backgrounds.
* Well-developed interpersonal, written, and time management skills, with the proven ability to work within a team environment and coordinate others.
