Overview

War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services include 24-hour inpatient care, Monday to Friday outpatient departments, iREAP Day Rehabilitation program, several specialist community teams including Transitional Aged Care, Geriatric Flying Squad, Young Onset Dementia, and a day centre program, 24-hour accommodation for regional guests and the full range of hospital support services. It is set within tranquil grounds and is a well-supported team environment.

The TAC program is a goal oriented, time-limited program that provides multidisciplinary support and low-level therapy to older people at the conclusion of a hospital episode, who require more time and support to complete their restorative process, optimise functional capacity, finalise and access longer term care arrangements. It operates both in the community and residential TAC settings. The TAC team comprises Social Workers, Physiotherapists, Occupational Therapists, Nurses, Dietitian, Speech Pathologists, Podiatry and Allied Health Assistants.

This role will help facilitate the delivery of an effective, flexible, innovative and integrated service. The role requires participation in quality initiatives within the TAC teams and/or hospital. A demonstrated commitment to quality improvement, evidence based practice, ongoing clinical and professional learning is required.

This position will join the WMH Transitional Aged Care (TAC) team to plan, coordinate and deliver high quality Social Work intervention to patients consistent with AHPRA, Uniting, NSW Health and South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards, to achieve patient/client health outcomes within specified timeframes.

This position is a Fixed Term Full-time role based in the Community TAC Program. Required days of work are Monday to Friday, 8.00am to 4.30pm. To commence ASAP through to 28 August 2026 with potential extension.

Selection Criteria

* Minimum of 2 years' experience working as a social worker and a high level of knowledge, expertise and skill in social work practice in an aged care and/or community setting
* Demonstrated ability to provide effective social work services to older people, including skills in psychosocial assessment and management, comprehensive client centered assessment, and care plan development for chronic and complex health care needs clients
* Demonstrated ability to provide comprehensive client centred assessment, outlining clinical reasoning, care plan development and case management in consultation with the client, carer and multidisciplinary team
* Demonstrated knowledge of community aged care services and the ability to assess, manage and provide support for clients with complex psychosocial issues and/or chronic complex health care needs
* Ability to work effectively both autonomously and within a multidisciplinary team and to exercise independent professional judgment on routine matters, commensurate with years of experience
* Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients across all levels and departments within WMH and external stakeholders
* Demonstrated commitment to quality improvement and evidence-based practice and a commitment to ongoing clinical and professional learning
* Degree or postgraduate qualification in social work and eligibility for registration with the Australian Association of Social Workers (AASW) AND Current Class C NSW Driver's license and willingness to drive as part of your role

All Category A NSW Health workers are required to have received an annual seasonal Influenza vaccination or have an approved medical contraindication certificate. COVID-19 vaccinations are strongly recommended for all NSW Health workers.
