Health Support Specialist
We are seeking a dedicated Health Support Specialist to join our team. The successful candidate will work closely with healthcare professionals to provide targeted support services for children and young people in care.
About the Role
* This is a 0.4 EFT fixed-term position until June 2027, working across our Werribee and Melton campus of Western Health.
The ideal candidate will have knowledge and experience working with Culturally and Linguistically Diverse (CALD) and Aboriginal and Torres Strait Islander children within the Child Protection system.
Key Responsibilities:
* Assist the Pathways to Good Health team in providing health management plans that meet the unique needs of children in care;
* Work effectively with families, carers, and other stakeholders to develop individualized support plans;
