Clinical Nurse Consultant – Integrated Chronic Care for Aboriginal People Program – Targeted
Position type: Permanent Part Time. Position Classification: Clinical Nurse Consultant Grade 2. Remuneration: $68.50 – $69.87 per hour + Super + Salary Packaging. Hours per week: 16. Location: Wonnarua Country – Cessnock District Hospital. Applications close: Sunday 25 January 2026.

Where you'll be working
You'll be based on Wonnarua Country at Cessnock District Hospital, working in a shared office with a close‑knit, family‑like team. You'll collaborate with an Aboriginal Health Practitioner, ICAPP CNC and a chronic disease RN.
What the role looks like
Support Aboriginal people across Cessnock, Muswellbrook, Singleton and Maitland. Provide clinical assessments, case management, care coordination and collaborate with local GPs and primary health networks.
Requirements
* Registered Nurse with current Authority to Practice with AHPRA
* 5 years full‑time equivalent post‑registration experience, including at least 3 years in this specialty field
* Approved postgraduate nursing qualifications in chronic disease management
