An exciting and rewarding opportunity is available for a suitably qualified Registered Nurse to join the Hospital in the Home team at Portland District Health.
The HITH nurse will provide high quality nursing services including assessment of the broader care needs to a culturally diverse community. The HITH nurse is responsible for improving the health status and independence of the patient and supporting their carer/s in their role, working as part of a team to balance workload and new patient referrals on a daily basis. The HITH nurse will be responsible to the delivery of comprehensive, person-centred nursing care, including higher acuity and new specialty clinical pathways as part of our HITH expansion. Days of work include Monday to Friday and weekends.
About you:
* Current Unrestricted Nursing Registration with APHRA
* Previous experience preferred but not essential
* Competent with PICCs and Ports, familiar with VACC dressings
* Well-developed interpersonal skills
* Ability to work as part of a diverse team.
* Demonstrated initiative and self-direction, and the ability to work autonomously.
* Strong commitment to high quality care

* Proficient in Microsoft Office (Outlook, Word, Excel, and Power Point), Adobe Acrobat, and social media platforms.
* Proven ability to handle confidential matters with discretion and liaise with a broad range of staff and stakeholders.
