We have a 6‑month position with full‑time hours, Monday‑Friday, for our Home Care team during our growth phase.
What’s involved in the role
* Assist in providing services tailored to individual clients’ needs, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultation with key stakeholders.
* Responsible for care management of existing clients and client‑related KPIs.
* Ensure care plans are up to date and clients receive appropriate care that corresponds with their needs, approval, and funding.
* Provide professional and competent day‑to‑day delivery of services in line with service expectations, and ensure staff understand each client’s needs and preferences.
What do you need to apply
* Certificate IV in Aged Care and at least 2 years of relevant industry experience.
* Knowledge of the Home Care Packages Program, Support at Home, Commonwealth Home Support Programme and Aged Care Quality Standards.
* Experience operating effectively in a multi‑site environment.
* A current driver’s licence and willingness to travel between HammondCare’s clients, sites, and services (salary package may include a motor vehicle).
* Strong administrative and computer skills.
