We are looking for an experienced and dedicated Case Manager to join our team at Momentum Collective. As a Case Manager for Homelessness Services, you will play a vital role in supporting and advocating for individuals within the community who are experiencing homelessness or at risk of homelessness.
**Responsibilities:**
* Provide culturally appropriate case management and support services to people and their and families experiencing homelessness.
* Develop strong, trusting relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
* Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
* Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
* Conduct comprehensive assessments of clients' needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
* Maintain accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines.
* Participate in regular supervision sessions, team meetings, training programs, and professional development activities to enhance skills and knowledge relevant to the role.
This is a part time fixed term role 3 days per week to 30th June 2026.

ABOUT YOU
* Cert IV Community Service and or other relevant qualification and or case work experience in an associated field
* Current Driver's Licence
* Satisfactory Criminal record check.
* Current Working with Children Check (NSW)
* Understanding and experience of working with Aboriginal people and their communities.
* Demonstrated experience in case planning and coordination of people with complex support needs.
* Understanding of, and demonstrated ability to work with Human Service Agencies.
* Demonstrated experience and or understanding of Trauma Informed Care.
* Excellent communication skills, particularly in the areas of negotiation, facilitation, mediation and problem solving.
* Intermediate computer literacy skills including Microsoft office suite and databases.
