About The Role:
An exciting opportunity exists for a motivated and enthusiastic Mental Health Professional to join the Continuing Care Team within Manning Mental Health Service, based at Forster Community Health Centre. This role sits within an acute community mental health service supporting consumers across the Lower Mid North Coast. As a key member of our multidisciplinary team, you will provide high-quality assessment, care coordination, intervention and follow-up for adults experiencing mental health concerns. You will work collaboratively with consumers, their families/carers, GPs, Community Managed Organisations and other service providers to ensure safe, effective, recovery-focused care.
Key Responsibilities:
Provide assessment, care planning and contemporary mental health practice.
Work in a consumer, family and carer-focused way.
Understand and apply trauma-informed care principles.
Communicate effectively and build strong therapeutic and professional relationships.
Work effectively within a multidisciplinary team.
Manage competing clinical demands and maintain professional boundaries.
Consult with GPs, CMOs and other service providers.
Commit to ongoing learning, evidence-based practice and service improvement.
Adopt a positive, collaborative and flexible approach to the team.
Be able to drive in NSW and interstate, and willing to travel for work-related purposes.

Requirements:
Recognised and accredited tertiary qualification and registration (or eligibility for) with the relevant board via AHPRA, or eligibility for membership of the relevant association – in one of the following disciplines: social work, psychology, occupational therapy, dietitian or nursing.
Additional Information:
Full-time position; part-time arrangements will be considered.
