Care Service Coordinator Healthcare | 6‐week contract | Bowen Hills (based onsite)$43.75 per hour
We are seeking experienced Care Service Coordinators to join a healthcare organisation on a short‐term 6‐week contract based in Bowen Hills. This role is suited to candidates who are comfortable managing a portfolio of clients, coordinating services, and supporting care delivery in a fast‐paced, service‐driven environment.
Key Responsibilities
* Act as the primary point of contact for a portfolio of healthcare clients
* Coordinate and schedule care and support services to meet individual client needs
* Manage inbound and outbound calls, providing clear and empathetic support
* Maintain accurate case notes and client records across multiple systems
* Support onboarding of new clients and provide ongoing coordination support
* Monitor service delivery and escalate issues as required
* Work closely with internal teams and service providers to ensure continuity of care
About You
* Experience in service coordination, case management, or account management roles
* Background in healthcare, aged care, disability, or community services preferred
* Comfortable working in a call‐centre style, high‐volume environment
* Strong customer service and communication skills
* Ability to manage sensitive conversations with professionalism and empathy
* Strong organisational skills with the ability to manage competing priorities
* Confident using multiple systems and technology platforms
