Your New Role
* Utilise your specialised social work skills and knowledge to ensure excellence in care for our patients.
* Responsibilities include caring for patients with a new diagnosis, transitioning care from paediatric to adult service, life‐prolonging dialysis, kidney donation, traveling from Tasmania for a kidney transplant, accompanying the team to a regional hospital clinic to assess transplant suitability, and providing end‐of‐life advice.
* Work with a dedicated and experienced social work team within the nephrology department.
* Opportunity for continuity of care across inpatient and outpatient settings.
About You
* Qualifications acceptable for membership with the Australian Association of Social Workers.
* Ability to work collaboratively with a large multidisciplinary team.
* Excellent interpersonal and communication skills.
* Skills in comprehensive assessment, discharge planning and counselling.
* Understanding of relevant legislation and practice framework relating to family violence.
