Join DACD as a GP – Central Coast (1 Day/Week)

We're currently seeking a General Practitioner to support our Central Coast DACD patients one day per week. This opportunity is perfect for a GP who values meaningful work, enjoys being part of a collaborative team, and is looking for flexibility in a supportive and purpose-driven environment.

Your Role
* Provide GP consultations to new and existing patients.
* Support ongoing chronic disease care planning and clinical care with home visits in the area and telehealth
* Liaise with care managers, pharmacists, and allied health professionals.
* Contribute to continuous improvement of care processes, health outcomes and patient experience.
What You'll Bring
* AHPRA General Registration (PGY5+)
* Minimum 2 years clinical experience in Australia
* Passion for holistic, patient-centred complex care
* Flexible to travel within the territory which covers Central Coast & Newcastle
* Able to attend virtual weekly MDT on a Monday morning for an hour
