POSITION DETAILS
The Key Support Worker delivers essential support under the Queensland Community Support Scheme (QCSS), assisting individuals under 65 years old (or under 50 years for Aboriginal and Torres Strait Islander people) who are not eligible for the National Disability Insurance Scheme. The role provides low-intensity support to help clients live independently, connect with their local community, and reduce social isolation.
Through a person-centred and strengths-based approach, the Key Support Worker empowers individuals to improve their physical and emotional well-being, increase independence, and access appropriate supports and services. This role is integral in enabling clients to safely manage their daily lives, participate in their communities, and reduce their reliance on high-intensity services.
RESPONSIBILITIES
* Following and providing clear support delivered within an individual support plan developed by the link worker.
* KSW will provide short-term and tailored services to enhance self-management and independence within clients' homes, including domestic assistance, meal preparation, personal care to enhance client capacity and promote self-management.
* Support people with disabilities to gain skills to be able to self-advocate, and ensure privacy and personal choices are respected at all times
* Ensure a person-centred active support approach to all areas of daily living of the people we support including their active engagement in the planning of all activities.
KEY SELECTION CRITERIA
Essential
* Relevant Tertiary Qualification, e.g., Community Services, Human Services, Social Work, or related discipline and/or relevant experience
* Relevant Certificate in community services or significant progress towards such a qualification and demonstrated knowledge/experience of the provision of community-based support services which reflect respect, inclusion, equality, and a strengths-based approach.
* Demonstrate knowledge and or experience working with diverse client groups including people with a disability, and their families/carers, people with mental illness, people who are homeless or residing in marginalised housing and who may be at risk of homelessness.
* Demonstrate knowledge and experience in delivering community-based support.
* Current class C driver license.
* National NDIS Worker Screening Check or current Yellow Card
Desirable
* Current First Aid and CPR certificate
* Understanding of Human Services Quality Framework (HSQF)
