About the Role:
As an EN / Care Coordinator, you will:
* Coordinate and oversee in home care services
* Conduct care assessments, reviews, and service planning
* Collaborate with clients, families, and the care team to ensure individual needs are met
* Monitor care delivery to ensure quality, safety, and compliance
* Support carers and staff with guidance, training, and updates
* Willing to travel wide bay and surrounding areas (Company car provided)

You will ideally have:
* Previous experience in Aged Care, Community Care, or Disability Support
* Strong time management and organisational skills
* Excellent communication and interpersonal abilities
* Knowledge of care standards, assessments, and service coordination
* A current driver's licence and access to a reliable vehicle
* A genuine commitment to "Improving the quality of life for those we serve"

Qualifications
* AHPRA Registered and practicing
* Drivers Licence
* Any other Aged Care / Health Care related certificates
* Proven experience in aged care, disability, or mental health within a community setting.
* Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
* Current First Aid, CPR certification, and National Police Check.
* Working With Children (WWC) check or willingness to obtain.
* A valid driver's license and access to a reliable car with comprehensive insurance.
