What You’ll Do
Become a key part of our Pottsville Pharmacy team as a fully integrated Aged Care On-Site Pharmacist! You will provide expert medication management to improve the health outcomes of residents in our partnered aged care facilities. You will work closely with general practitioners, nurses, and facility staff to build trusted relationships with residents, their families, and healthcare professionals.
Support the Aged Care facilities in achieving high standards of medication safety, providing quality use of medicines (QUM) activities, and attending Medicines Advisory Committee (MAC) meetings.
Monitor aged care residents’ medication to ensure optimal outcomes and address medication-related issues promptly.
Work closely with healthcare teams to facilitate transitions of care, manage high-risk medications, and conduct regular medication reviews.
What You’ll Bring
Bachelor of Pharmacy or equivalent with current Pharmacist Registration with AHPRA
High level of computer literacy including systems knowledge of dispensing software
Minimum 2 years’ experience in a community pharmacy setting is required.
Completion of credentialed pharmacist training as an Aged Care On-Site Pharmacist is desirable.
It is mandatory to provide:
Proof of immunisation against infectious diseases
A national police check within the last 12 months
