About the Position:
We are seeking a dedicated and compassionate individual to work with clients and their families in their homes, enabling them to maintain their independence.
Your Key Responsibilities Will Include:
* Providing support to clients in their community, who have been referred for assistance at home
* Developing and implementing services and programs that meet the unique needs of each client
* Fostering strong relationships with families and carers, acknowledging their individual requirements
* Working closely with clients to set achievable goals, taking into account their strengths and challenges
* Adopting best practice approaches and collaborating with a multidisciplinary team to deliver high-quality care
* Maintaining accurate records using client management systems
What We Need From You:
* Ability to work independently in the community
* Excellent verbal communication skills
* A patient, empathetic, and caring nature
Requirements:
* Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management) or equivalent knowledge and experience
* Experience in aged care or transferable skills will be highly regarded
* Demonstrated ability to work within established guidelines and achieve service delivery objectives
* A valid driver's licence
* Intermediate level of computer skills
* Effective time management and organisational skills
* Ability to work collaboratively as part of a team
