Role Overview
We provide a new approach to support transitional care. The role is based in the community service department with close ties to acute inpatient services. It involves both inpatient and community-based care, including remote care.
Responsibilities
* Support elderly patients transitioning from acute inpatient to appropriate settings.
* Coordinate inpatient, subacute, ambulatory, and community home-based care.
* Provide remote care and rapid follow-up access for patients.
* Ensure clinical care suitability for elderly patients in various settings.
Qualifications
* Fellowship of the Australian and New Zealand College (FRACP).
* Experience in transitional care management for elderly patients is preferred.
Location
Victoria - 1 hour flight from Melbourne.
