Clinical Nurse Consultant Palliative Care Job Description
The role involves providing comprehensive assessments, diagnoses, and tailored treatment options directly to patients, whether in-person or through Telehealth.
Collaborative planning is a key aspect of this position, working with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences.
Empowerment through education is also essential, offering health education and support to patients, families, and care staff, ensuring optimal outcomes and understanding of care processes.
Community connections are crucial, building strong partnerships with Community Palliative Care services, GPs, residential care staff, and community services and families to manage patient care effectively, reducing avoidable hospital admissions.
Adaptability and leadership skills are required, evolving practice to meet the needs of our service and taking an active role in professional development and leadership within the Palliative Care team.
This role requires current endorsement as a Registered Nurse by the Nursing and Midwifery Board of Australia via AHPRA.
A post graduate qualification in Palliative Care is preferred/working towards Masters qualification.
Advanced clinical knowledge and skills in Palliative Care are necessary, as well as advanced interpersonal, communication (written and verbal) and problem-solving skills.
Knowledge, understanding and experience in research and quality improvement are also required.
