About the Role

Key Responsibilities:
1. Build strong connections with clients and their families
2. Assign staff to clients based on qualifications and availability, considering client preferences
3. Organise support for clients with disabilities and monitor progress
4. Refer clients to agencies for additional help
5. Assess client needs and resources for health, welfare, and other facilities and services
6. Respond promptly to emails, enquiries, and requests from clients, staff, and management
7. Maintain accurate records of incidents and feedback as required
8. Reorganise work priorities to meet the needs of a fast-paced environment
9. Support all clients to meet their goals
10. Conduct Client Care Assessments and Care Planning
11. Provide clinical guidance to other team members for client management
12. Discuss and promote Right at Home's services with medical professionals and referral sources
Requirements
To succeed in this role, you'll need:
1. Current AHPRA registration as a Registered Nurse
2. Experience in assessing and planning care needs
3. Understanding of the Aged Care and Disability systems
4. A strong client focus based on their goals, preferences, and needs
5. Current National Police Check (or commitment to obtain prior to start)
6. NSW Working with Children Check (or commitment to obtain prior to start)
7. NDIS Worker Screening Check (or commitment to obtain prior to start)
8. NSW Full Drivers Licence
9. The right to work in Australia
10. Organisational, administrative, and time management skills
